Patient Portal Information

Please click the Patient Portal button below to complete the registration form.  Once completed, you will receive a confirmation email that it has been received.  A staff member will reach out to you within 2 business days about next steps and if we have a counselor that meets your needs.  If you do not hear back from a staff member within that timeframe or need additional assistance, please contact the office at 603-356-6400 to verify that we have received your registration and/or to answer your questions.  Business hours are Monday through Friday from 8:00 AM to 4:00 PM.

Please note that completing the registration does not guarantee that our counselors have openings.
